Transitions Mental Health Association
The San Luis Obispo County Behavioral Health Integration Project (B-HIP) Steering Committee is comprised of county agencies, community health centers, hospitals and a Medi-Cal managed care plan. The collaborative is leading primary care and behavioral health integration efforts throughout the county by building provider capacity to address the needs of behavioral health patients and supporting peer navigators to help bridge the gap between the supply and availability of care and the demand and utilization of services.
The collaborative may serve as a resource to others interested in pursuing similar work as it continues to develop expertise in these areas:
Hosting community-wide "grand rounds" trainings on empathy, behavioral health integration, and bridging treatment gaps for patients with mental health and substance use disorders
Enhancing data collection on patient experience
Expanding the use of a universal release-of-information form to enable data exchange among providers
Education and certification for peer navigators and new trainings for providers
Gaining consensus among stakeholders on a set of countywide metrics to measure progress in advancing behavioral health integration and participating in the Dashboard Learning Community Cluster
Conducting a community capacity assessment
