Advancing Primary Care and Behavioral Health Integration through Community Collaboration
Community Clinic Association of San Bernardino County
The Community Clinic Association of San Bernardino County collaborative, comprised of county agencies, community health centers, behavioral health providers, and a Medi-Cal managed care plan, led the primary care and behavioral health integration initiative in San Bernardino County. They completed an in-depth analysis of the current status of the county’s three tier needs, and identified strategies to address partnership expansion, behavioral health and primary care capacity building, issue and population prioritization, and referral and care coordination processes among others. They also engaged service providers and networks to explore use of telehealth as an integration tool, and explored replication of existing model integration programs, namely the Inland Empire Health Plan Provider Portal and the Department of Public Health’s Integrated Psychiatry Program. Resultantly, the Inland Empire Behavioral Health Integration Initiative took shape, which is piloting projects that support care coordination and behavioral health integration.
