Health Quality Partners of Southern California

This collaborative, based in San Diego, is led by Health Quality Partners of Southern CA (a subsidiary of Health Center Partners of Southern California). There, they partner with local community health centers, mental health agencies, alcohol and substance-use service providers, and the Behavioral Health Services Administration of the County Health and Human Services Agency. The collaborative has been focused on developing and piloting new care-delivery processes that support providers in working together to improve transitions between systems of care and coordination of treatment across systems of care for patients.

The collaborative may serve as a resource to others interested in pursuing similar work as it continues to develop expertise in these areas:
 

Shared care planning between primary care, mental health, and substance use providers in separate locations. Protocol available here.

Care transitions and advance planning and support for patients - for adults, children and youth - who are transitioning from a county mental health provider to a federally qualified health center that can serve as their long-term behavioral health and primary care home. Protocol available here.

 

Collecting data on the social determinants of health that impact patients with behavioral health conditions, and building partnerships with local social service providers who can assist in meeting patients' non-medical needs.
