Health Alliance of Northern California (HANC)

Health Alliance of Northern California served as the convening agency for the Shasta Health Assessment and Redesign Collaborative (SHARC) and the Behavioral Health and Substance Abuse Prevention Committee, comprised of county agencies, community health centers, behavioral health providers, and a Medi-Cal managed care plan, to lead a primary care and behavioral health integration initiative in Shasta County. SHARC completed a needs assessment which informs systems level strategies to maximize existing resources and align county-wide initiatives, as well as a network map which has improved coordination within the local delivery system. These tools have prompted efforts to redesign services through community collaboration and training. Practice level strategies were also integrated into service provider settings such as embedding substance use counselors into care teams, establishing co-occurring disorders groups, implementation of the Screening Brief Intervention and Referral to Treatment (SBIRT) model into primary care settings, and creating access to regional psychiatric consultation among primary care providers.
