All Californians -- particularly those who are underserved and/or low income -- deserve the chance to live a full and healthy life in safe and connected communities. Making that possible for individuals who struggle with mental health conditions, substance use, and live in economically distressed communities with fragmented support systems, will require significant collaborations and re-imagination of the systems that serve those most in need. Together as changemakers, every individual involved in this initiative is leading the way towards a vision of integrated and equitable health care that benefits people, families and communities all across the state.
