DASHBOARD LEARNING CLUSTER
The goal of this Learning Cluster is to create a composite “dashboard” tool of metrics that communities might be able to use to assist them in: identifying behavioral health needs in their communities; and, measuring, on a consistent and regular frequency, the metrics to determine if progress is being made at improving the behavioral health access and outcomes for a community. CARE COORDINATION LEARNING CLUSTER
The goal of this Learning Cluster is to promote integration and care coordination in localities across California by providing resources and support to improve cross-provider and sector data sharing. Key activities include: provide coaching, legal consultation, and facilitate peer-to-peer learning and information exchange through webinars. Through grantee sharing of experiences and promising practices, we will surface a range of issues that affect cross-system data sharing and care coordination, and promote strategies that address challenges to collaborative work in communities.
